Popular Mechanics has a article titled: "CORPS SAYS LEVEES NOT READY FOR KATRINA'S WAVE, SOIL EROSION LEADING CULPRIT" about why the levees broke in New Orleans. It sounds to me that it was a result of typical bureaucratic missteps. Nothing really wrong, on its own, it was just a cumulation of these small screwups.
